Register With The Practice?

If you move into the practice area (see map page) you may apply to register with us. Please collect registration forms and a sample bottle from the surgery and book an appointment with our Health Care Assistant. All new patients over 18 will need to provide the following information:

1. Completed registration forms,
2. Medical card if available,
3. Proof of identity with photograph,
4. Proof of current address less than 3 months,
5. Proof that you are ordinarily resident in the UK,
6. Urine sample.

You may start booking appointments with doctors/nurses 48 hours after your initial visit.

How Do I....
Change My Details?

It is important to let us know of any change of name, address or telephone number so that we can contact you if we need to and also notify the Primary Care Support Service who keep records on behalf of the Health Authority.
